Liability Waiver and Release of Claims
Any physical or contemplative activity carries the possibility of injury, physical or psychological. Yoga, breathwork, somatic practice, hypnotic journeying, and the other activities offered at TaKun Space are not a substitute for medical or psychological diagnosis or treatment. By participating, I acknowledge and assume the risks involved.
I am participating in Ignite Your Inner Fire at TaKun Space on Saturday, May 16, co-facilitated by Shana Bell and Nina Mizuno.
I am aware of the physical and emotional risks involved with this activity and understand it is my personal responsibility to consult with my doctor or mental health provider regarding my participation. I have no medical or psychological conditions that I am aware of which would prevent me from taking part, and I assume responsibility for any risk or injury I may sustain as a result of my participation. I understand that it is my responsibility to find a pace that suits me.
I release Shana Bell, TaKun Space, Nina Mizuno, Trinity Mystic, and their employees and agents from any responsibility or liability concerning the application, processing, and consequences of any activity I elect to participate in, including but not limited to yoga, breathwork, hypnotic journeying, somatic activation, kundalini-inspired movement, t’ai chi, Reiki, and any other workshop or activity offered in this space. I consent to have the above described services applied.
Understanding the risks involved, I release Shana Bell, TaKun Space, Nina Mizuno, Trinity Mystic, their employees, and their agents harmless against any and all liability, damage, or expense arising out of or in connection with actions, claims, or damages resulting in personal injury or disability, physical or psychological, or the transmission of a communicable disease that I might incur as a result of the services provided. I agree to voluntarily participate, understanding these risks and their outcomes. I affirm that, understanding the activities described above, I am healthy enough to participate.
